I, the parent or legal guardian of the child stated above, understand that my child will be taking part in a kids yoga class through SoulJuice. I understand that yoga requires physical movement and body exertion which can result in accident or physical injury. I accept, that any yoga instructor, nor the hosting facility, SoulJuice, is liable for any injury, accident, damage to the student stated above or their belongings. I, the parent/guardian, understand that yoga cannot be used as a substitute for medical attention, diagnostics or treatment. I should consult a physician prior to starting this yoga program. I, the parent/guardian, recognise that it is my responsibility to inform the instructor of any illness or injuries before every yoga class. I will not preform any yoga postures that cause pain or strain. I acknowledge that for the Growth section of the SoulJuice Kids Yoga programmes my child will be participating in a shared video project. On this basis, I acknowledge and give consent that any video/photo content may be shared publicly (where appropriate) and be used for the purposes of SoulJuice marketing collateral and training purposes.
